In order to complete this form you will need the following information:

  • Your agreed climb date and route
  • Your passport information
  • Details of your emergency contact
  • Your Doctor's details including surgery address

Passport Information

To travel you must have a valid 10 year passport that runs for at least 6 months after your return to the UK. If it doesn’t you will need to apply for a new passport as soon as possible. Please forward details to The Nasio Trust as you receive these.
